Opening & getting started

When does Pacora open?

We open September 1, 2026 in Mission Valley, San Diego. You can book a virtual Meet & Greet now or join the waitlist to be among the first to establish care.

Are you accepting patients yet?

Not quite yet — we open September 1, 2026. Before then, you can book a virtual Meet & Greet to learn how we work, or join the waitlist so you're first to know when enrollment opens.

What is a virtual Meet & Greet?

A free, no-pressure video conversation with our team. It's a chance to see whether Direct Primary Care is right for you, ask questions, and learn exactly what's included.

How does the waitlist work?

Joining the waitlist reserves your place and means you'll be among the first we contact when enrollment opens ahead of our September 1, 2026 launch. Because our panel is intentionally small, joining early is worthwhile.

The basics

What is Direct Primary Care?

Direct Primary Care (DPC) is a simple way to receive comprehensive primary care through one flat monthly fee. In return you get longer appointments, easy access to your care team, and no per-visit insurance billing. It's a return to relationship-based care, with time to get to know you and your health goals.

How is this different from a regular doctor's office?

In a traditional office, the practice bills your insurance for each visit and often cares for a very large number of patients, which is why appointments can be short and hard to book. By keeping our patient panel small, we're able to offer longer appointment times, same-day or next-day access, and direct communication between visits.

Is Direct Primary Care the same as concierge medicine?

No. Both emphasize personalized care and better access to your provider. The difference is how care is delivered: concierge practices generally bill insurance in addition to charging a recurring fee. In Direct Primary Care, one flat monthly fee covers your routine primary care and eliminates insurance billing for office visits, which lets the practice prioritize accessible, relationship-based care. Pricing

How much does care cost?

Pacora charges a flat $100 per month for patients 18 and older. Your care plan covers comprehensive primary care, with no separate charge for office visits. If a service falls outside what's included, we'll always discuss any additional cost with you upfront.

Do you offer a college student rate?

Yes. We offer a college semester option at $75 per month, locked in for the semester, based on your proof of enrollment. It's designed to help students stay connected to primary care while they're away at school — for an illness, a prescription refill, or a convenient telehealth visit. Proof of college enrollment required.

What's included in my care?

Your care plan covers the everyday care most adults need: annual wellness visits, chronic disease management, preventive care and cancer screening, women's and men's health, weight-loss management, nutrition and lifestyle medicine, mental and behavioral health, and minor wound care. It also includes telehealth for California patients and direct communication with your care team between visits. See what's included →

Is there anything that costs extra?

Some in-office procedures — such as certain injections, cryotherapy, and more involved wound care — carry an additional charge because they require extra supplies or materials. Whenever that's the case, we'll discuss the cost with you and get your OK before we proceed, so there are never surprise bills.

Are there copays?

No. There are no copays for your visits, and no surprise bills. Your flat monthly fee covers your primary care.

Can I cancel if it's not right for me?

Yes. We believe in earning your trust every month, so there's no long-term contract — we simply ask for advance notice to cancel. [PLACEHOLDER — confirm cancellation notice period (e.g. 30 days) and whether any re-enrollment fee applies if a member rejoins later.]

Is there a sign-up or registration fee?

You pay your monthly fee — simple and predictable. [PLACEHOLDER — confirm whether a one-time enrollment/registration fee applies; if so, add the amount here.]

Insurance & how DPC fits in

Is Direct Primary Care health insurance?

No — this is important. DPC is a healthcare model for primary care services; it is not health insurance and does not replace it. It covers your everyday care, not hospital stays, surgeries, specialists, or emergencies. The most common approach is to pair DPC with a catastrophic or high-deductible health plan: your primary care is handled here, while insurance helps cover unexpected major medical expenses.

I already have good insurance — is DPC still worth it?

It can be. Even with excellent coverage, it may still take weeks to get a routine appointment, and visits can feel rushed. DPC complements your health plan with convenient access to your care team, same-day or next-day appointments whenever possible, longer visits, a provider who gets to know you, and easy communication between visits. You continue using your insurance for services outside our practice — specialists, imaging, hospital, and emergency care.

Can I use my HSA or FSA to pay?

As of January 1, 2026, federal rules allow many people to use HSA funds toward qualifying DPC fees. Because FSA reimbursement rules can vary by employer plan and administrator, we recommend checking with your benefits administrator or tax advisor before using FSA funds.

Can I still use insurance for labs, imaging, or prescriptions?

Yes. When you need care outside our office, you can use your health plan just as you normally would. We can send prescriptions to the pharmacy of your choice and orders to the lab or imaging center you prefer; those third-party providers will bill your insurance for their services. [PLACEHOLDER — confirm whether discounted/wholesale in-house labs & medications are offered, and we'll add member cash-price savings here.]

Getting care

How do I reach my provider?

Easily — that's the whole idea. You can reach your care team by phone, message, or telehealth, and book in-person visits at our Mission Valley office. [PLACEHOLDER — confirm preferred contact methods (patient portal, text, email) so we can name them specifically.]

Can I get a same-day or next-day appointment?

We strive to make care available when you need it. Thanks to our intentionally small patient panel, we're frequently able to offer same-day or next-day appointments, so you can spend less time waiting and more time getting answers.

Do you offer telehealth?

Yes. We offer telehealth visits for patients located in California, so you can get care from home, work, or your dorm when an in-person visit isn't necessary.

What happens if I need a specialist or hospital care?

Your insurance covers those, and we'll help coordinate. If you need a specialist, we'll guide you and share the notes needed for a smooth handoff.

What should I do in an emergency?

For any medical emergency, always call 911 or go to your nearest emergency room. DPC is not a substitute for emergency care. Once things are stable, we're here to help with your follow-up care.

Eligibility

Who is a good fit for Direct Primary Care?

DPC is a great fit for a wide range of people: those who are self-employed, freelance, or run a small business; anyone without insurance who wants affordable, predictable access to a provider; people who want more time with a provider who will get to know them; and students who want quality care on a semester timeline. If you value easy access and time with your provider, DPC is worth a look.

Do you treat children?

Currently, we care for adults 18 and older. Pediatric care is something we're excited to offer in the future. If you're interested, join our pediatric interest list and we'll let you know as soon as it's available.

Can I join if I have Medicare?

Yes. Medicare patients at Pacora are cared for by Derek Rogoz, PA-C. You'd sign a simple private agreement that neither you nor the practice will bill Medicare for your DPC care, and you keep your Medicare for everything else — specialists, hospital care, labs, and imaging outside our office. If this applies to you, reach out and we'll walk you through exactly how it works. [PLACEHOLDER — confirm Medicare opt-out stance before publishing.]
